Please arrive 15 minutes before your first appointment to fill out paperwork. If you’d like to fill out forms we're happy to email them to you, just give us a call or drop us a line at vertex1clinic@gmail.com

• We provide loose fitting shorts and a t-shirt for your appointment

• Feel free to bring your own gym clothes if you prefer

• Please bring a valid driver’s license and insurance card

• We accept all major credit cards, cash and checks

All cancellation or appointment changes can be requested via telephone at 0208 371 0267 or email vertex1clinic@gmail.com.

A cancellation fee of £100.00 will apply for appointments that are cancelled with less than 24 hours notice.

You will receive an email confirmation the day before your scheduled visit to remind you of your appointment. This will give you an opportunity to reschedule if necessary without penalty.
